Basic information used for your homepage copy, local SEO schema, and Google Business Profile.
1.1 Legal practice name *
1.2 Preferred display name on website (if different)
The name you use with clients day-to-day, if shorter or more conversational than your legal name.
1.3 Practice type *
1.4 Primary phone number *
1.5 Primary contact email *
1.6 Full street address *
Must exactly match your Google Business Profile. If mobile or telehealth only, note that instead.
1.7 Years in practice
1.8 Do you offer telehealth / virtual consultations?
1.9 Office / appointment hours *
All days, all session types. Used in schema markup and your contact page.
1.10 In one or two sentences, what do you do and who do you serve?
Write this as you would say it to a new client — conversational, not clinical.
1.11 What is your personal philosophy or approach to health?
This becomes your About page narrative. Alternative health clients choose a practitioner because of their philosophy — be specific about what you believe.
02
Website goals
What does this website need to accomplish? These answers determine what we build first and where we focus the copy.
2.1 What is the single most important thing this website should do? *
2.2 What does success look like 6 months after launch?
Be specific — "more clients" is not measurable. "4 new discovery calls per month from the website" is.
2.3 What are the top three problems with your current website? (or "no website yet")
2.4 Websites you admire — inside or outside your niche *
Share URLs and note what specifically you like — the photography, the tone, how they explain complex concepts simply, their booking flow.
2.5 Websites you strongly dislike or want to avoid resembling
03
Credentials & authority
Alternative health clients are credential-checking before they book. This section determines how we build your authority and trust signals — the most important conversion factor in this niche.
Important: I display credentials exactly as you provide them. I am not responsible for verifying that your listed credentials are current or accurately described. Please confirm all designations with your licensing body before submitting.
3.1 List all your professional credentials, degrees, and certifications *
Include the full designation and the full name (e.g. "L.Ac. — Licensed Acupuncturist, Virginia Board of Medicine"). List each on its own line.
3.2 Are you a member of any professional associations?
These appear as trust signals on your site. Include the full name and your membership status.
3.3 Have you been published, quoted in media, or spoken at events?
Published articles, podcast appearances, conference talks, media mentions. These become authority signals on your About page.
3.4 Do you offer continuing education or teach other practitioners?
04
Your ideal client
Alternative health clients are often self-directed researchers who have already tried conventional medicine. Knowing exactly who you serve shapes every word on the site.
4.1 Describe your ideal client in detail
Age range, health situation, what they have already tried, what they are searching for, what they are afraid of, what they need to believe before they book.
4.2 What objections or hesitations do new clients typically have before booking?
These become the objections the website copy addresses directly — on your homepage, FAQ, and About page.
4.3 Do you serve any specific populations?
4.4 What geographic area do you serve?
05
Services, products & content
What pages do we need and what do you already have? This section builds your site map.
5.1 List all services and programs you want on the website *
Each major service typically gets its own page for SEO. List everything — we decide together what merits a dedicated page.
5.2 Do you sell supplements, products, or digital programs? *
5.3 Which pages do you need?
5.4 Do you have existing written content?
5.5 Do you have professional photos of yourself and your space?
5.6 Do you have client testimonials or results stories we can use?
First names only. No specific health outcomes or before/after claims without FTC-compliant disclaimers — we will discuss this in Section 8.
5.7 Do you have an existing logo?
06
Design & brand
Your visual direction. Even rough answers save significant revision time.
6.1 How do you want your website to feel?
6.2 Existing brand colors
Hex codes if known, or describe: "sage green and warm cream," "deep forest green and gold."
6.3 Colors you absolutely do not want
6.4 Any other design notes, preferences, or things to avoid?
07
Technical & integrations
Existing accounts and tools that need to connect to the new site.
7.1 Do you have an existing domain name? *
Domain name and registrar (GoDaddy, Namecheap, Squarespace, etc.)
Do you have login access?
7.2 Existing hosting and access level
7.3 What booking tool do you currently use (or want to use)?
Unlike conventional medical practices, most alternative health practitioners do not need a HIPAA-compliant booking tool unless they are a covered entity. We will confirm this in Section 8.
7.4 Tools and integrations needed
7.5 Do you have an active Google Business Profile?
08
Compliance & legal
Alternative health websites carry specific compliance risks around FTC health claims, FDA supplement regulations, and state scope-of-practice laws. Your answers determine how we write and structure the site.
Your responsibility: I build and publish the content you provide. I am not a lawyer and cannot advise on whether your services or claims comply with FTC, FDA, or Virginia state licensing requirements. If any question below raises uncertainty, please consult a healthcare attorney before we launch.
8.1 Are you a HIPAA covered entity? *
If you are an MD, DO, NP, PA, or licensed therapist billing insurance, you are likely a covered entity. Health coaches, nutritionists, personal trainers, and most acupuncturists are typically not — but confirm with your attorney if unsure.
8.2 Virginia licensing status — confirm which applies to you *
This determines what language the site can legally use to describe your services.
8.3 Do you or will you use before/after photos or results-based testimonials?
8.4 Do you sell or promote supplements or nutraceuticals on the site?
8.5 If you offer IV therapy, confirm your supervision arrangement
8.6 Legal pages you can supply copy for before launch
09
Timeline & final notes
Deadlines, decision-makers, and anything else I should know before we start.
9.1 Is there a specific date you need the site live by? *
9.2 How quickly can you turn around feedback on design drafts?
9.3 Who needs to approve the website before it goes live?
9.4 Who will manage the website after launch?
9.5 Preferred communication during the project
9.6 Interested in a monthly maintenance plan after launch?
9.7 Anything else I should know before we start?
